To support patient admissions in the Mississippi region, the full-time Clerical Territory Patient Admission Coordinator will establish communication with clients and referral sources, manage the referral process, and ensure timely documentation and follow-up, working onsite during standard business hours with rotating weekends and holidays. Key responsibilities Collaborates with referral sources, sales teams, and local agency operations to gather necessary information for processing quality referrals Documents referral-related information and actions in the medical record accurately and in a timely manner Ensures prompt attention to referrals from various platforms and follows up with clients and caregivers to confirm or collect information Required qualifications High School diploma or GED Minimum of one year of experience in home care, hospice, or personal care services Prior experience with Electronic Medical Records (EMR), HCHB preferred Experience dealing with various payors in healthcare, such as Medicare and Medicaid, preferred Strong alphanumeric data entry skills required
