Patient Scheduling Coordinator
To support efficient patient flow, the full-time Patient Scheduling Coordinator will coordinate and manage patient visit schedules for Home Health and Hospice disciplines in a remote capacity, ensuring timely initiation of care and compliance with regulatory guidelines. Key responsibilities Coordinate and manage patient visit schedules, ensuring adherence to CMS and regulatory requirements Maintain ongoing patient schedules, adjusting visits based on various factors such as clinician availability and patient needs Collaborate with Intake, Clinical Leadership, and hospital partners to facilitate smooth transitions from referral to Start of Care Required qualifications Associate degree in business, health administration, or a related field, or equivalent experience required Two years of scheduling or customer service experience required Basic knowledge of computer applications required Previous healthcare experience preferred Experience with Microsoft Office applications preferred
