A 70-year-old in Ohio opened her mail in October and found the letter every Medicare Advantage member dreads: her plan would no longer be offered in the county for 2026, and her coverage would terminate December 31. She had a consequential choice to make. What she did next saved her from one of Medicare’s most expensive traps.
She returned to Original Medicare and bought a Medigap Plan G policy under a federal guaranteed-issue right. No health questions. No denial because of the atrial fibrillation in her chart.
Why the 63-Day Window Matters More Than the Premium
Most people compare Medicare Advantage and Original Medicare by looking at monthly premiums. At age 70, reversibility may matter more. The one-time federal Medigap Open Enrollment Period lasts six months, beginning the first month someone is 65 or older and enrolled in Part B. After it closes, insurers in most states can examine an applicant’s health history, charge more, or refuse coverage. A cardiac diagnosis, recent cancer treatment, insulin use, or a hospitalization can make approval difficult or impossible.
A Medicare Advantage plan nonrenewal creates one of the narrow openings back into Medigap without underwriting. The protected application period begins 60 days before the Advantage coverage ends and closes 63 days afterward. Medigap coverage itself cannot begin until the Advantage plan terminates.
The right applies when an Advantage plan leaves Medicare, stops operating in the enrollee’s area, or the beneficiary moves out of the plan’s service area. It is not a general opportunity available whenever someone changes their mind, although separate trial rights and state protections can apply in other circumstances.
The Cost of Waiting
In 2026, the standard Part B premium is $202.90 per month. A Medigap Plan G premium for a 70-year-old might run approximately $130 to $250, depending on ZIP code, sex, tobacco status, carrier, and pricing method. That puts the combined cost near $330 to $455 per month before standalone Part D drug coverage and any income-related Medicare surcharge.
What does Plan G buy her? It covers the $1,736 Part A hospital deductible, the $434 daily hospital coinsurance for days 61 through 90, and the $217 daily skilled-nursing coinsurance for days 21 through 100. After she meets the $283 annual Part B deductible, it also covers most remaining Part B cost-sharing.
She gains nationwide access to providers who accept Medicare, with no Advantage-plan network. Original Medicare can still require prior authorization for certain services, including limited demonstration programs, but Plan G removes the insurer-controlled authorization rules attached to an Advantage plan.
Another Medicare Advantage plan could still be the less expensive choice. She would retain an annual in-network out-of-pocket maximum, although Part D drug spending follows a separate limit and out-of-network coverage depends on the plan. The difference is what happens later. If she chooses another Advantage plan and the event-specific Medigap window expires, a future return to Original Medicare may once again require underwriting.
The Trap She Sidestepped
Two mistakes commonly waste this protection.
The first is treating the nonrenewal letter only as an invitation to choose another Advantage plan. That may still be the right decision, but it should come after pricing the Medigap opportunity. The federal guarantee is available only if she returns to Original Medicare. It does not protect a Medigap application while she remains in another Advantage plan.
The second is counting from the wrong date. The window does not begin when the October letter arrives. It opens 60 days before the existing coverage ends and closes 63 days afterward. With coverage ending December 31, the application period extends into early March. Applying before January 1 is still cleaner because it allows the new coverage dates to be coordinated without a gap.
State law may create additional openings. New York and Connecticut provide especially wide access, while Massachusetts, Maine, and several other states have their own enrollment protections. The state insurance department or State Health Insurance Assistance Program can identify the rules that apply locally.
What to Do When the Letter Arrives
Two actions carry most of the weight.
- First, compare another Advantage plan with Original Medicare, a protected Medigap option such as Plan G, and standalone Part D coverage. Plan N is not included in this particular federal guaranteed-issue right unless state law provides additional protection.
- Second, submit the Medigap application during the protected period and attach the plan’s nonrenewal notice. Keep a dated copy as proof of eligibility and obtain written confirmation before the window closes.
The 2026 Social Security cost-of-living adjustment (COLA) added roughly $56 a month to a $2,000 benefit, while the standard Part B premium rose by $17.90. A Medigap premium remains a substantial additional expense. The difference is that it is known and budget worthy. A denied application after the next cardiac event is neither.
