A House Has Been Bought Under My Mom’s Name And Signature Over 15 Years Ago And We Never Knew About It Until Now

In 2011 someone used my mom’s ID, Social Security number, and her signature to buy a house and we never knew about it until the creation of this post. My mom tired to apply for healthcare benefits and we’ve only lived in one house our entire lives.
Insurance says she owns two properties and that they sent her a letter regarding to applying for the insurance. My family never knew of this and now they want my mom to confirm or deny that the property is hers.
We got the papers saying it’s under my mom’s name and even has her signature which she has never sign for. My mom has gone to notaries and an office of the company that built the house. Neither could tell us who we can ask for help in identifying if she is the owner of the house.
How can we know who made this decision and how can we prove that my mom never agreed to buy a house in the first place?
My parents were separated in 2010 and my mom was taking care of me and my two siblings. They got back together in 2011 when my dad was paying off the house we currently live in. I would say early 2011 because the paper says she signed for the other house in August of 2011.
