How Can I Refuse Treatment In Advance If Decision-making Capacity Is Impaired?

I know there is DNR (Do Not Resuscitate) but this is different than that.
If you go or are taken to the hospital and are delirious for whatever reason (neurological, metabolic, severe pain etc) if you do not want treatment, there is a high chance you will not be listened to because you are not “sound of mind” or you just won’t be aware enough to say something. That’s not a medical condition where resuscitation is even considered.
I have a chronic illness and while I am not yet diagnosed, there has been small neurological occurrences and other events that make me fear I could lose awareness and be given treatment when I don’t want to be. Even if you think that’s irrational, the point is that I still want my medical wishes to be respected.
What kind of legal-medical steps can I take aside from a DNR to refuse treatment in advance— and make sure that this is viewed and respected at the time of treatment?
