Long Story, But I Need Advice

In June of 2022 I lost my job, and within 6 months, knew I could not work any of the available jobs in my area due to a medical issue, so I just worked Door Dash to keep food in my kitchen and utilities on. I informed a debtor of this, and without any contact, they wrote off, then sold, the debt.
In October of 2025 I appeared before. JP in my county about this, and stupidly, as I was focused on an upcoming surgery and prep for that, I agreed to just wait and not speak before the judge when the plaintiff attorney suggested it. In early Jan of 2026 I received a summary judgement, stating I had not appeared for a 2nd hearing, which I was never notified about.
Now to the rest. In July of 2025 "document service" happened, on a date when I was not home and can prove it. The document server. "described me" and listed that it was me by using my name, describing a person who was bald (I am not and was not then,) shorter, and much lighter, than I am. This indicates that they lied, and officially stated it on legal papers.
So, what should I ask the legal advice office and what should I include in my motion for a new trial?
