Home Care Costs Increase 3% Nationally, Wyoming Most Expensive State

The national median hourly rate for private duty nursing delivered in the home is $90.

That’s according to the latest Cost of Care Survey from CareScout, a wholly owned subsidiary of Genworth Financial that helps people find providers and navigate the long-term care landscape. The company on Monday released the findings, which were drawn from more than 25,000 rates gathered from various types of long-term care providers between July 2025 and November 2025.

This is the first year that CareScout has reported private duty nurse rates. The median per-visit rate for private duty in 2025 was $160.

The median rate for non-medical home care increased 3% compared with 2024 levels, reaching $35 an hour.

“At that rate, the annual cost totals $80,080, assuming 44 hours of care a week,” CareScout noted in a press release. “The increase aligns closely to broader economic trends, with inflation averaging 2.7% and wage growth at approximately 2.5% in 2025.”

The median monthly rate for assisted living increased 5%, meaning that annual costs now total around $74,400.

The median daily rate for nursing home care increased 2% for a semi-private room and 1% for a private room, bringing the annual median costs to $114,975 and $129,575, respectively.

Adult day bucked the trend of cost increases, with the national median daily rate declining 5% to $95 a day. Five days of adult day services a week would therefore come to about $24,700.

Of course, costs vary from state to state. Wyoming came in as the most costly state for both non-medical home care and private duty nursing in the home.

CareScout had insufficient data for Alaska, Arkansas, Connecticut and Louisiana to include them in the state-by-state rankings for private duty nursing.

In terms of other care settings, Hawaii led the pack for assisted living ($145,065 median annual cost), Oregon was the costliest for a private nursing home room ($221,372 median annual cost), and Alaska was the costliest for a semi-private nursing home room ($333,975 median annual cost). Adult day services were most expensive in Montana ($69,395 median annual cost).
