Got Rear Ended By A Semi

Location: Washington state

Like the title says, was driving my work truck on I5 and was rear ended while coming to a stop I filed a police report and drove away from. The accident the thirdparty paid out my boss for the damage to the truck (10k), after a year of diagnostics and consultations I need anterior & posterior L5 L6 fusion and disk replacement on S7

The surgeons that recommended the work quoted in the $300k range for everything just for medical costs of the work to be done, not counting the medical costs already incurred (55k), not counting lost time, ect.

The third party's first offer came back at 120k, it is a commercial insurer

My case worker seems to be trying to close my case quickly and manage my expectations. What am I looking at here, realistically. I just want to get what I need to fix my body when the time comes, I'm 33 and have two little kids, I can't do a year (if not longer) break from my obligations,

Edit: I have a lawyer, I have had the same caseworker since I hired them and the case worker was the one who guided me and my wife through building my case and making appointments for specialists, she no longer works with the firm. My new caseworker is the one who seems to be attempting to manage expectations and close the case
