Guardianship Over Adult With Mental Disability

I have an adult sibling with schizophrenia. He was diagnosed 20+ years ago, and over the years would often go off his meds, end up in psychosis, become a danger to himself and end up in the psych ward for months at a time. He's mentally regressed to about age 4. He does not take care of himself - will not wash unless made to, won't clean up after himself (food to washroom messes), won't change his clothes unless made to, etc He is on disability but will give all his money away to people till he has nothing for groceries. He has been in the hospital for 6+ months and they are looking into releasing him.
He refuses to take his medication when he's out, and will just end up back in within a year, he has had a few instances where he has almost died due to not taking his meds.
What would be the next steps for guardianship or POA? We would like to get him in a home, or at least a psw/social worker for him for a few hours a day but he won't agree to it.
