Lied On A Patients Fmla Paperwork

Hi everyone,

I put myself in a sticky situation here and I need to know what to do.

Im a nurse, and I work in primary care. I have a patient, A, who's taking care of her sick mom. A requested 4 days a week of FMLA because her mom needs to go to appointments four days a week and she needs to drive her there and home.

The doctor only signed off on one day per week because she misread the notes and thought it was supposed to be one day per week. When I realized the doctors error, I fixed the paperwork. But instead of getting the doctor to sign off on the paperwork again, I just faxed the paperwork. A needed it asap as it had already been a week. I dont really know why I did this, so please dont ask me why I did it.

I need to know: if I fess up, is this a firable offense? Is there more legal damage that could be done to me? I am pretty sure im just going to talk to my supervisor about it at work, but i am hoping someone might have some advice for me as I am up here at three am sorta freaking out about it. Thanks!
