Obtaining My Deceased Mother’s Medical Records
Location: Minnesota.
My mother passed in 2025. I am attempting to obtain her medical records pertaining to her death (extended stay in ICU), diagnosis and subsequent treatment (ALS, less than 1 year of records), and other pertinent primary care records leading up to her diagnosis.
There was no probate and is no need for it. Her estate is settled already. So there is no legal representative. I am her adult daughter. But I am hitting roadblocks. I have emailed so I have in writing the three medial systems I am requesting records from and I am getting generic responses that don’t even answer my question on how to obtain her records.
I have explicitly told them there is no legal representative as there was no probate. And I have explicitly asked what forms or further information they would need from me. But all I have gotten is “legal documentation”. The hospital where she died also said they needed a death certificate and I’m like ???? she literally died there???
What do I do here?
