Phone Number Stolen And Identity Lost

I recieved a text from my phone company 5 days ago that a SIM change request was requested for my mobile phone account. The next thing I knew my personal phone lost connection and went into SOS mode. Luckily I had a work cell phone and was able to call my phone carrier and request that my SIM be returned to its original state. I was able to get my phone working within an hour and didnt think too much about it after all was well. So fast forward an hour and Im finishing my task at work I take a look at my personal cell phone again and I see more texts about my SIM being changed and the SOS instead of connectivity bars is being displayed. I called my carrier again and was told my phone number was switched to a new SIM again and my phone number was deleted from my account. At this point the alarm was raised. I checked my email and noticed password change requests for all of my financial and email apps. I immediately called my wife and told her to help me call our financial institutions but it was too late. The hackers cloned my phone and through the cloud had access to all of my data. For days Ive been on the phone with banks, credit card companies, email carriers and so on. These hackers currently still have access to my phone number and access to all of the apps that were open on my phone. They have all of my personal info now. They have opened accounts to place orders for electronics and other things using my financial info. There have been hard inquiries into my credit and much much more. I also know that the SIM was changed to a carrier out of state. What are my options when dealing with my phone carrier? How can somone call a phone carrier and simply migrate a persons phone number twice to a phone out of state without raising red flags with the carrier?
