Supervised Visitation

Location: Montgomery County Texas

I (dad) have custody of my son, 3M. His mother left when he was less than a year old. Within the last 6 months, we got a support order through the AG. She does not pay, which whatever, I dont need anything from her. Her visitation is sporadic. Supposed to be 1st, 3rd & 5th weekends. I want to get her visitation rights changed to supervised. She is in a relationship and has another child with a man that there is now a protective order against for a situation that happend last week. There was talk of him abusing her previously but now there is proof. She is trying to get the order and charges against him dropped. I am concerned for my son's safety if he goes to their apartment-- which she refused to tell me the apartment number to. He has other charges, some dismissed and some convictions- drugs & weapon. Current situation is still pending. She didnt pick our son up this last weekend, thankfully.

Question boils down to-- is it unreasonable to request supervised visitation? And would I do this through the attorney general or court system?
