Mom Impersonating Me To My Medical Providers

Hi, everyone.
I (20F) really need some advice. I recently discovered that my abusive mother has been calling my doctor and pretending to be me. I think she is doing this to get access to my records since I did not sign an ROI, so I am the only person who should have access to the info legally. I received visit summaries from Telehealth calls/meetings when I did not participate in any. I’m very upset What are my options here? I would obviously like to stop her from doing this and if possible secure some kind of protective order against her. I also have evidence of a number of other instances when she violated my rights & even committed crimes against me, including theft, stalking, and threatening to commit suicide “right now” while driving with me in the car. Any advice here would be much appreciated. And I do not want to hear “just avoid her”, that’s not helpful. Also, has she committed a crime in impersonating me to access my medical records and meet with my doctor?
